What is mobile outpatient therapy?
Mobile outpatient therapy is one-on-one therapy services performed in the client’s home versus traveling to an outpatient clinic. Clients receive personalized care in the privacy of their home.

How do I pay for mobile therapy?
Available for Medicare Part B beneficiaries, Private insurances, and Out-of-network/cash based. Medicare covers 80% of charges and if you have a secondary insurance it is covered at 100%

How do I start outpatient services?
A referral from your doctor for outpatient therapy is needed to start with Habit Project Mobile Therapy.

How to submit a referral?
If you are a medical practice interested in submitting a referral for a patient or a individual/caregiver desiring services but have questions on our process, you can reach out to us anytime at (352) 462-0889 or Admin@habitproject.rehab. We will do our best to get back with you in a timely manner to answer your questions and introduce you to Habit Project. If you do not have any questions, you may Fax your referral/ prescription for Physical and Occupational Therapy directly to us at (352) 559-2364.
